Forms include the Document Declaration Form, individual run sheets and the Driver Public Assessment Form.
The new v16 version (May 2025) of the Associates’ Logbook can be found here. It no longer contains a Document Declaration Form, which can now be downloaded using the link above, or multiple run sheets. Extra copies of these can be found using the link above.
IAM RoadSmart have produced a document for Associates describing the Advanced Driving test in detail. GAM have edited this down to nine pages to make it more digestible for Associates: it can be found here.
Unfortunately, IAM RS have not included the ten course handout sheets in the new version of the Associate’s Log Book. GAM have repeated them here via an IAM RS link for ease of access. The ten handouts are:
